厚生労働省2004年10月に改革のグランドデザイン案を示した。内容として、障害保健福祉施策の総合化、保護中心から障害者のニーズと適正(適当で正しいこと)に応じた自立支援型システム(system)への転換(切り替え)、制度の持続(継続)可能性の確保などがあげられた。これを具体化したのが2005年制定の障害者自立支援法である。

The Ministry of Health, Labour and Welfare presented the Grand Design for Reform in October 2004. The contents included integrating measurements for health care and welfare for people with disabilities, converting (switching) from being protection-centered to a system that supports independence according to the needs of people with disabilities, and appropriateness (being proper and correct), and securing sustainability(continuation) of the system. This (proposal) was realized as the Services and Supports for persons with Disabilities Act established in 2005.
