In-Home Service Plan
要介護者が在宅で生活を送るために、介護支援専門員が、本人や家族の同意を得ながら作成する、介護サービス（long-term care service）に関する計画のこと。生活上の課題を明確にして、サービスの種類、内容、時間などを決めていく。
