Individual assistance plan/Individual care plan
介護サービス計画のこと。ケアマネジャーが、利用者個別の状態に合わせて、各専門職の視点から、目標や具体的な支援内容と方法を示す。
This refers to a long-term care service plan. The care manager presents the goals and specific support contents and methods from the perspective of each profession to suit the individual states/conditions of the clients.
