介護保険法に基づいて、地域に暮らす高齢者の健康や生活維持のために、市町村に設けられる施設のこと。保健師、社会福祉士、主任介護支援専門員が必ず置かれて、連携しながら、広い範囲の相談に乗っている。
This refers to a facility established by the municipality to maintain the health and livelihoods of elderly people living in the community, based on the Long-Term Nursing Care Insurance Act. Public health nurses, certified social workers, and chief care managers must be stationed there, and be able to give advice on a wide range of areas in coordination with each other.
