Long-Term Care Support Specialists
介護保険制度における専門職であり、ケアマネジャーとも呼ばれる。要介護者が心身の状態に合わせた介護サービスを利用できるように、家族も含めた相談に乗って、ケアプランの作成や関係機関との連絡調整を行う。
This is a specialist job under the long-term nursing care insurance system that is also called ‘care manager’. They give advice to persons requiring long-term care and their families on how to be able to use services that match their physical and psychological states, create care plans, and/or contacts and coordinates with relevant organizations.
