Preventive Long-Term Care Support
対象は自宅で生活する要支援者である。その人に合ったサービスを受けるために、専門職が介護予防サービス計画を作成する。また、計画に基づく支援が提供されるように介護予防サービス事業者との連絡調整も行う。
